The Silent Killers: Chronic Kidney Disease and Heart Failure
Chronic kidney disease (CKD) is a stealthy condition, affecting over 35 million Americans, or 1 in 7 people, according to the National Institutes of Health. It often goes unnoticed until significant damage has occurred, and its link to cardiovascular problems has been well-established. However, the exact reasons behind this connection have eluded scientists, partly due to shared risk factors like obesity and hypertension.
But here's where it gets controversial: researchers from UVA Health and Mount Sinai have identified a kidney-specific risk factor that could be the key to understanding this deadly link.
Unveiling the Culprit: Circulating Extracellular Vesicles
The researchers, led by Dr. Uta Erdbrügger, an internal medicine physician-scientist at UVA's School of Medicine, have pinpointed particles called "circulating extracellular vesicles" as the culprit. These vesicles, produced by diseased kidneys, carry small, non-coding RNA called miRNA, which are toxic to the heart.
In a laboratory study using mice, the researchers found that blocking these extracellular vesicles significantly improved heart function and alleviated heart failure. They also confirmed the presence of these harmful vesicles in blood plasma samples from CKD patients, providing strong evidence of their role in causing cardiovascular issues.
Dr. Erdbrügger explains, "Doctors have long wondered about the communication between organs like the kidney and heart. Our research shows that EVs from the kidney can travel to the heart and cause toxicity. We're just scratching the surface of understanding this complex communication."
